Room Set-up Form: Please use this form to indicate how Facilities should set up a room for your event. Please drop this form off at the office 10 days before the event. The form can also be faxed to (408) 366-6695.

Facilities Work Order Form: Please use this form to request any repairs, modifications, additions, installations, or any other work to be done at the PBCC facilities.

PBCC Driver Expectations (PDF): This document lists expectations and guidelines for those driving PBCC and private vehicles for church activities.

PBCC Driver Application (PDF): This form is for all PBCC employees and volunteers who will drive PBCC vehicles and/or their own or other private vehicles for PBCC activities and ministries. Only drivers who have been approved as "Church Drivers" are permitted to drive PBCC vehicles and vehicles loaned to PBCC, and only drivers who have been approved as "Volunteer Drivers" are permitted to transport minors in their own vehicles for PBCC ministries and activities.

Single Use Driver Form (PDF): This PDF form may be used when a need arises for transportation when the time frame precludes the normal application process and/or if the driver will most likely only drive for a single event.

Vehicle Loan Form (PDF): From time to time folks in the congregation allow PBCC to use their vehicles for PBCC activities. This form needs to be completed by each person loaning a vehicle for PBCC activities.

Vehicle Request & Vehicle Use Forms: Please use the PBCC Vehicle Request form to request use of a PBCC vehicle. This form should be submitted to the Facilities Manager. The Vehicle Use form is to be filled out for every trip made in a PBCC vehicle, and turned in when the keys are returned. These forms are available in each vehicle.

Youth Release Form: Parents (or guardians) need to fill out one of these forms for each child taking part in a PBCC-sponsored youth event or activity away from the church facilities. The activity or event leader should have these available during the entire event. The release form is good for only one local event. Youth leaders should use an event-specific release form for activities out of town.


Child Safety: Procedures have been established at PBCC to protect our children from any form of abuse. Everyone who works with our children are screened and trained for the protection of the children on our campus.

PBCC/PBCWG Youth Ministry Application:
All volunteers and staff working with minors at PBCC need to fill out a Ministry Application and agree to a background check. The application and background check is part of PBCC's child safety program. Please fill out the form online by clicking on: PBCC/PBCWG Youth Ministry Application
